What is a CMS Certification Number (CCN)?
A CMS Certification Number (CCN) is a unique number assigned to certain types of providers โ most importantly:
Hospitals
Other facilities such as dialysis facilities, rural health clinics, and federally qualified health centers
Hospice agencies
Home health agencies
CCNs are sometimes referred to as Medicare Provider Numbers.
How are hospital CCN Numbers defined?
Some hospitals have separate CCNs for certain units, including psychiatric units, rehabilitation units, and swing-bed units. These CCNs will have a letter in the third position, see below.
EXAMPLE:
21-0101 - ABC Hospital
21-T101 - ABC Hospital Rehabilitation Unit
The following alpha-characters in the third position as indicated:
M - Psychiatric Unit in Critical Access Hospital
R - Rehabilitation Unit in Critical Access Hospital
